Design side, it’s a quite long work but not so difficult, but the two smaller dials on the left can’t be used to show time in any time zones, they can only show time with prefixed difference from the primary time.
…which means you have to tell the designer what time zones you want before the watch will be done.
Yes and no. Designer can preset them, but as the code is inside an editable XML file, the final user could modify them at his own will.
